Bottom line
Testosterone is about far more than sex. It takes part in maintaining muscle, bone density, red-blood-cell production, and sexual function. It is a main beam of male health 4.
Low testosterone is not something one lab slip can declare. Endocrine Society guidance asks for symptoms confirmed by two morning fasting labs before a diagnosis is on the table 11.
Erectile function is a cardiovascular warning light. Urologist Rena Malik cites a figure: among men who develop erectile dysfunction, about 15% have a cardiovascular event within seven years 6.
For most people the first rank is not treatment. It is the foundation. Obesity is the main risk factor for functional secondary hypogonadism, and substantial weight loss can reverse that state in some men 7.
The evidence on testosterone replacement therapy, TRT, is milder than early pictures of it. A large randomized trial found it not inferior to placebo for cardiovascular events, while atrial fibrillation, acute kidney injury, and pulmonary embolism occurred more often. It helps sexual function. The gains in vitality and physical performance are limited 8910.
What it is
Testosterone is the main male sex hormone. Both sexes make it. Men make much more. It is produced in the testes, under orders from LH, luteinizing hormone, secreted by the pituitary. When output is high, negative feedback tells the brain to ease off 12.
Once testosterone enters the blood, most of it is carried in a box. A little over half is bound to albumin, and close to half is bound to sex hormone-binding globulin, SHBG. Only a small share is free. What can actually act is the free part plus the part that comes off albumin easily. Together those are called bioavailable testosterone 4.
Testosterone is also a precursor. In the body it can be converted to the stronger DHT, tied to hair loss and the prostate, or converted to estrogen. Estrogen has physiological jobs in the male body too. A level that is too low actually affects libido and cognition 5.
Testosterone is already at work before birth. Evolutionary biologist Carole Hooven explains that embryonic testosterone shapes body and brain in the male direction, and that early hormone waves affect behavioral tendencies across a life. Rare cases of 5-alpha-reductase deficiency let scientists see what testosterone and DHT each govern 3.
So-called low testosterone, hypogonadism, means symptoms and labs both point to insufficient output: fatigue, a drop in libido, worse erections, low mood, loss of muscle, plus confirmation on a morning fasting lab 11.
Why it matters
A slow fall with age is agreed, and the speed differs by person. A urology review lists bone density, body composition, red-cell production, and sexual function as reasons clinicians pay attention 4.
Obesity is the more urgent variable right now. A clinical review lists obesity as the main risk factor for functional secondary hypogonadism, and reports that substantial weight loss can reverse the state in some men. The problem is fat tissue interfering with the feedback between testosterone and estrogen, not a broken gland 7.
Erectile dysfunction is especially worth taking seriously. Malik calls it the canary in the coal mine. The vessels of the penis are finer than those of the heart. Endothelial dysfunction often shows here first, and cardiovascular events come years later. Her practice is to send men with true erectile dysfunction for a cardiovascular evaluation 6.
Urologist Mohit Khera, speaking in an interview, names another sore point. Neither side likes to open the topic. Clinicians don't ask about it. Patients are embarrassed to ask about it. Many people endure it in silence 2.
There is a psychological layer too. A review writes that testosterone affects more than libido and erection. It also affects energy, mood, and even confidence and competitive drive 5.
Different perspectives
Main positions
One site files hormones and male health together, with more than one long episode and essay 1. The diagnostic view matches the guidance and stresses individualization. Low testosterone is symptoms plus a trustworthy lab, not one line for everyone. The Khera episode says symptoms and blood values are read against each other 2.
Before any medicine, the house is put in order. Cardiovascular risk and blood pressure are not left unaddressed while exogenous testosterone is rushed in. After treatment starts, monitoring covers prostate enlargement, acne, hair loss, and physiological levels, and the drug is stopped if the benefit is unclear 8.
On the cardiovascular cloud over TRT, there is a specific reading of the TRAVERSE trial. In that randomized trial of men who already had cardiovascular risk or disease, testosterone gel was not inferior to placebo for major cardiovascular events. That result is a relief. Atrial fibrillation, acute kidney injury, and pulmonary embolism were more common in the drug group, and monitoring cannot be skipped 89.
On the old worry about prostate cancer, the episode with Khera specifically discusses the popular claim that TRT raises prostate-cancer risk, and sorts the evidence 2.
The evolutionary line is another thread. In the conversation with Hooven they talk about how prenatal testosterone shapes the male body and brain, about differences in aggressive style between the sexes, and about the two faces of competitive drive in evolution. Suppress it, and the output it brings is lost as well 3.
Another site defines male sexual health as fertility, hormone health, erectile function, and libido taken together, and it says plainly that this is an indicator of overall health 5.
The first-line practices on that topic page are all lifestyle. Regular exercise, managing stress, less smoking and less alcohol, less environmental exposure to bisphenol A (BPA) and phthalates, and not leaving the scrotum hot for long stretches. All of these are tied to sperm quality and hormone status 5.
In the episode with Rena Malik, the link between erectile dysfunction and the cardiovascular system is stressed again and again. The two also discuss pelvic-floor function, how nerves and blood supply support sexual function, and which common drugs drag on libido 6.
The figure Malik passes on is worth keeping. Among men with erectile dysfunction, about 15% have a cardiovascular event seven years later. She treats erectile dysfunction as an early signal of vascular health, not merely a small urology complaint 6.
A further public record has two pieces on testosterone.
One is a conversation with Derek, of More Plates More Dates. Testosterone is not an on-or-off state. Body composition and environmental exposure both affect hormone output and signaling, and the effect differs by person. The core argument is obesity. Extra fat changes the feedback between testosterone and estrogen, and losing fat may let the balance come back 7.
In the same piece a systematic review is cited that found only a weak association between vitamin D status and total testosterone, with large differences between studies. Both may simply be mirrors of overall health. That is the standing reservation about narratives in which one nutrient pulls testosterone up 7.
The other piece is a clip of an interview. It explains why the definition of low testosterone is symptoms plus labs, and what is done in clinic. That content lines up with the statements above 8.
No target number is offered that fits everyone. The frame stays the same: put the changeable factors — body composition, sleep, environmental exposure — on the table first 7.
Where views agree and differ
The views agree closely on the large direction:
- Diagnosis runs symptoms and labs in parallel. A single number is not a diagnosis 811.
- Obesity, sleep, exercise, smoking and alcohol, and environmental chemicals are the first tier of influences. Weight loss comes before medication 57.
- Erectile dysfunction is treated as an alarm for vascular health, and a cardiovascular look follows 26.
They differ on the treatment threshold. One account uses symptoms first, an individualized trial, close monitoring, and a stop if there is no benefit 28. The guidance is more conservative: symptoms plus two morning fasting total-testosterone labs, and free testosterone when SHBG is off 11. Another side weights behavioral tools and the equipment layer of pelvic floor and vessels 56. A third stresses that group numbers miss individual differences, and that environment is often underestimated 7.
Oral testosterone-boosting supplements, tongkat ali and the like, are another split. One show discussed them. One view has said plainly that tongkat ali was never put forward as something for everyone 6. Evidence cited elsewhere shows a weak link between micronutrients and testosterone 7. Another account keeps lifestyle ahead of supplements 2. No account here treats these products as a foundation plan.
PearHealth's synthesis
The order we take: look at symptoms first, then a morning fasting blood draw, then a repeat after an interval to confirm. Once a diagnosis holds, people who carry extra weight lose weight first, people who sleep poorly repair sleep first, and people short on movement start moving. Only then is TRT weighed with a clinician 711.
Our attitude to TRT is that the benefit has a shape and the risk has a list. Improvement in sexual function is the most solid finding in randomized trials. Vitality shows almost no measurable benefit. Physical performance sits between the two 10. Cardiovascular events overall are not inferior to placebo, but the signals for atrial fibrillation, kidney injury, and pulmonary embolism call for regular monitoring, and a rise in hematocrit is an item to watch 89.
We do not take the story that higher testosterone is better, and we do not take the story that TRT is inherently dangerous. The treatment is a tool that needs two hands. Symptoms, labs, the cardiovascular baseline, and a willingness to be followed: all four, then it is picked up.
No natural food or supplement has been shown to stand in for that sequence. The evidence for testosterone-boosting supplements is out of proportion to the advertising 7.
How strong is the evidence
The evidence on this topic is fairly easy to sort, because two large randomized trials are in hand.
Randomized trials, groups differing only in the thing under study:
- The TTrials randomized 790 men over 65, with testosterone on the low side and with symptoms, to testosterone gel or placebo. After one year, sexual activity, libido, and erectile function improved significantly. Improvement on the six-minute walk was significant only in a pooled analysis. A vitality scale showed no benefit 10.
- TRAVERSE enrolled men who already had cardiovascular risk or disease. Testosterone gel was not inferior to placebo for major cardiovascular events. Atrial fibrillation, acute kidney injury, and pulmonary embolism were more common in the drug group 89.
Observational studies, which cannot prove cause:
- The link between obesity and low testosterone, and reports of partial reversal after weight loss, come from clinical reviews and cohorts. The direction agrees, but randomized-trial-level evidence of a weight-loss intervention is missing 7.
- The link between erectile dysfunction and later cardiovascular events is follow-up data. The mechanism, endothelial dysfunction showing first in small vessels, is plausible and is not closed at the level of an individual 6.
Clinical experience (case observations, not a formal trial):
- A description of settle the cardiovascular picture first, try the medicine, stop it if it does not help is a frame from clinical experience. It has value, and it is not a trial result 8.
Guidance, a society's systematic sorting of the evidence:
- Endocrine Society guidance uses GRADE to describe evidence quality. The diagnostic part — two morning fasting total-testosterone measurements, and free testosterone when SHBG is off — is hard content a clinic can actually use 11.
What you can do
Directions many people take:
- If the waist is high, lose weight first. Obesity is among the most reversible causes, and losing fat may let testosterone come back 7.
- Strength work a few times a week, enough total activity, and a fixed sleep time. Late nights drag on the hormone axis 5.
- Less smoking and alcohol, and less everyday exposure such as heating food in plastic 5.
- A sustained drop in libido, fewer morning erections, fatigue, and a lower mood: write them down with sleep and weight for a clinician 2.
- With erectile dysfunction, urology plus blood pressure, lipids, and glucose 6.
- Leave testosterone-boosting and testicular-support products off a self-directed list. The blood test comes first 7.
Questions worth raising with a clinician:
- Do my symptoms look like low testosterone? Is a morning fasting total testosterone, repeated once on another morning fast, worth doing 11?
- My SHBG is high or low. Could total testosterone be misleading, and is a free-testosterone measurement worth adding 11?
- I am considering TRT. What are my cardiovascular risk, hematocrit, and PSA baseline, and what is the follow-up plan 8?
- For my erectile problem, is the first look at the vessels rather than only a drug 6?
- I am taking certain medicines for chronic disease. Could they affect sexual function 6?
Common misconceptions
Misconception one: testosterone is only a sex hormone, and only someone who can't needs to pay attention. It also runs muscle, bone density, red blood cells, and mood. Can't is only one signal when it goes wrong 45.
Misconception two: one low number on a lab slip equals low testosterone. The guidance asks for symptoms plus two morning fasting labs. A single draw, an afternoon draw, or a draw during illness can all be off 11.
Misconception three: TRT means boundless energy and a return to age twenty. In randomized trials the improvement in sexual function is clear. Subjective vitality barely moves on the measures used, and the physical gain is limited too 10.
Misconception four: TRT certainly harms the heart, or it has been proved completely harmless. TRAVERSE's answer is not inferior to placebo. Signals for atrial fibrillation, kidney injury, and pulmonary embolism are a reminder that monitoring cannot loosen 89.
Misconception five: tongkat ali, maca, and similar natural testosterone boosters are harmless and effective. Tongkat ali was discussed on the show, and one view there said it had never been put forward as fitting everyone. No party treats it as a foundation plan. The review cited shows a weak link between micronutrients and testosterone. Natural is not the same as tested 67.
How it connects to your blood work
Male hormone labs are sensitive to timing, method, and repeats. This section says, roughly, what each item looks at.
- Total testosterone (total-testosterone): the total, and where diagnosis starts. Morning fasting, then a second morning fasting draw 11.
- Free testosterone (free-testosterone): unbound and able to act. Key when the total is borderline or SHBG is off 411.
- Sex hormone-binding globulin (shbg): the carrier. Close to half of testosterone binds to it. More carrier can leave the total looking fine while the usable share is not 4.
- Estradiol, sensitive assay (estradiol-sensitive): estrogen made from testosterone. The topic page notes a physiological job in male health too 5.
- Luteinizing hormone (lh): the pituitary's order to the testes 12.
- Prolactin (prolactin): one episode tied this pituitary hormone to dopamine and sexual function. It is also listed when low testosterone is assessed 12.
- Prostate-specific antigen (psa): a baseline before TRT, then follow-up. Khera discussed testosterone and the prostate in that interview 2.
- Hematocrit (hematocrit): the red-cell share. The guidance lists it before treatment and during follow-up 11.
