Bottom line
Anti-aging supplements are the layer of the longevity conversation with the loudest commercial volume and the thinnest human evidence. Across these views, they are placed after behavior 41.
The mechanism story for NAD+ precursors (NMN, NR, and others) is tidy. NAD+ falls with age. Taking a precursor does raise NAD+ in people, and it is well tolerated 11. Evidence of longer life is written in yeast, worms, and rodents. Human trials have not yet reported a lifespan endpoint 6.
Resveratrol is the cautionary tale in this trade. It became famous twenty years ago. Human randomized trials have kept failing since. One research summary calls the latest one another nail in the coffin 3.
Urolithin A has the most concrete human data of these lines. In randomized trials it is associated with gains in muscle strength, endurance, and mitochondrial markers. The studies are still small and short 1013.
A summary line from one long conversation is representative. No supplement has been shown to extend human life. The problem is that a human lifespan trial cannot really be done 5.
What it is
NAD+ (nicotinamide adenine dinucleotide) is a general cofactor in the cell. Energy metabolism, DNA repair, and immune activation depend on it. The sirtuin protein family and the PARP repair enzymes, both staples of longevity research, use it as fuel 6. The body regenerates most NAD+ through a salvage cycle. It can also be built from food raw materials: tryptophan, three forms of vitamin B3, and NMN, which is present in broccoli, avocado, beef, and other foods 6.
NAD+ precursor supplements turn those raw materials into a tablet. NMN and NR (nicotinamide riboside, present naturally in milk and beer) are the two star molecules 68. They differ in where they enter the NAD+ synthesis path. Conversion of NR depends on two kinases, Nrk1 and Nrk2. Nrk1 is in almost every mammalian cell. Nrk2 is found only in skeletal muscle, heart, and brain 8. A large animal literature shows that precursors improve a range of age-related conditions. On the human side, the first phase 1 trial of NMN was still underway at the time of that page 7.
Resveratrol is a polyphenol in red grape skin, and in the root of Japanese knotweed. A trace amount in red wine made it famous. It took off because it was found to activate SIRT1, one of the sirtuins, and sirtuins are thought to mediate the lifespan effect of calorie restriction. The story "resveratrol equals a calorie-restriction mimetic" was told for many years 9.
Urolithin A has a different origin. It is not a ready-made plant substance. Gut microbes make it from ellagitannins in pomegranate, strawberry, and walnut. It is a postbiotic 10. The catch is that not everyone's gut microbes make much of it. People sort into metabotypes that produce it, produce some, or produce almost none. Taking purified urolithin A directly bypasses that difference 10. Its signature mechanism is mitophagy: selectively dismantling broken mitochondria, then making new ones 10.
One frame has a slot ready for molecules you swallow. It divides longevity tactics into five domains. Supplements, drugs, and hormones share the exogenous-molecule domain. The frame offers three questions. What is the goal? Is there a marker to track and adjust by? Is the risk larger than the benefit 1?
Why it matters
Market size shows how far the misunderstanding goes. Years after the resveratrol story was questioned, countless related supplements and skin products are still for sale 3.
The gap between "the mechanism is tidy" and "the clinic sees a benefit" is invisible to an ordinary buyer. In an interview, San-Millán's example is blunt. Raising NAD in plasma does not mean NAD rose inside the cell and the mitochondrion, and it certainly does not mean a benefit for lifespan. He says plainly that these data need a cautious reading 2.
Real differences between people exist. Whether urolithin A works depends on whether your gut microbes are "producers" 10. NAD precursors are also distributed unevenly across human tissues 6.
There is an opportunity cost. The same money and attention, spent on training, sleep, and food, has a return that has been shown over and over. Spent on a "youth pill," the return is still an unknown 45.
Different perspectives
Main positions
One public frame for supplements is the exogenous-molecule domain inside a five-domain tactic, and that frame stresses that this domain is highly individual. Rather than "what to take," ask the goal, the marker, and the balance of risk and benefit first 1.
One view holds that, beyond disease-specific medications used by indication, prescription drugs such as rapamycin may have a broad effect at the level of lifespan under a geroprotection theory, and SGLT2 inhibitors are raised as a possible example too 5. Rapamycin is a prescription drug, not a shelf supplement 5.
On the NAD line, a more sober position is restated. Sirtuins, NR, and NMN are not to be tied, in logic or in practice, to extending lifespan. Raising NAD through NR does have some interesting favorable effects, including on inflammation 5.
The same account states the basic difficulty. A human lifespan trial cannot be done. The people who could wait for the result cannot wait that long 5.
On resveratrol, a 2024 article is blunt. This compound, which shot to fame about twenty years ago under an anti-aging label, has had its scientific basis questioned for a long time, and a new meta-analysis of randomized trials on metabolic health added another nail 3.
San-Millán, in an interview, also posed a sharp question. Tumor-cell glycolysis depends heavily on NAD. Could raising NAD on purpose help a tumor in some settings? An unpublished mouse pretest saw faster tumor growth in the group given NR. The sample was tiny, but the direction is worth watching 2.
A supplement topic page is clear. Supplements are part of a broader strategy, and that strategy starts with health behaviors and sound nutrition. Build the foundation first: sleep rhythm, morning light, regular exercise 4.
On method, that page argues for a single ingredient when the goal is clear, so the amount can be adjusted to the person's response. A basic multivitamin is a safety net, especially when the diet is limited. Adjust with season, state, and age 4.
The long conversation hosted there laid the trade's questions out. What NAD, NMN, and NR each are. Intravenous versus oral. Bioavailability and safety. Where FDA oversight sits. And the final question: can these things actually extend life 5?
That conversation sits in the camp of using some, ranked after behavior. Supplement lists were compared. The conclusion was that these are only possible additions to healthspan. Sleep, nutrition, and exercise are the trunk 5.
An NAD+ topic page gives the mechanism panorama. NAD+ is the cofactor for energy metabolism, DNA repair, and immune activation. It falls with age, and its depletion may push several age-related conditions 6.
The NMN topic sorts the data honestly. Animal studies improve a range of age-related outcomes. On the human side, the first phase 1 trial had only just started, and its goal was safety and bioavailability 7.
The NR topic explains the entrance difference, Nrk1 versus Nrk2, and the tissue distribution, and it notes that animal evidence is emerging 8.
The resveratrol topic keeps a more optimistic face and also gives the research detail. Small clinical studies observed metabolic changes resembling calorie restriction. The same page tells the story from 1992 onward 9. Read next to the 2024 meta-analysis, that is a full cross-section of the trade.
The urolithin A topic is the page with the thickest human data of these lines 10. In randomized trials, oral urolithin A is absorbed. Muscle biopsy shows higher proteins tied to mitochondrial quality control, improved muscle ATP production, and lower inflammation markers 10. In middle-aged and older participants, lower-limb strength rose by about 10 to 12 percent, and timed walking and cardiorespiratory markers improved as well 10.
Where views agree and differ
Four points of agreement. Supplements are not the main engine of longevity. Behavior is 45. NAD precursors raise NAD in people. Lifespan evidence is written in animal models, and a human lifespan endpoint has not been measured 611. Resveratrol's human record keeps sliding 3. Any anti-aging supplement goes through three questions first: goal, marker, risk against benefit 1.
The split is temperature. One platform presents mechanism and early human signals more favorably, especially urolithin A and NAD metabolism 107. Another line of commentary keeps pouring cold water, and it stresses unknown risk at the level of tumor metabolism 2. A middle position uses them, and holds the order and the rules for talking about evidence 45.
Another real split is whether it is worth trying. One frame is to write the goal down first, ask whether a marker can be tracked, and ask whether risk exceeds benefit 1. That frame has also been written to say that the absence of a marker is not the same as leaving it unused. It means watching the result in a more attentive way 1. Other views are more willing, once behavior is in place, to treat some supplements as adjustable tools 410.
PearHealth's synthesis
The reasonable posture in this trade is to read the evidence in layers. Mechanism research (NAD declining, mitophagy) is solid. Human safety and surrogate markers (raising NAD, muscle mitochondrial markers) are early but real. Hard endpoints — longer life, prevention of disease — are entirely absent 61113.
Our combined view: treat NMN, NR, and urolithin A as promising research directions, not as verified anti-aging tools. If someone truly wants to try, it is worth setting a measurable goal and a time limit with a clinician, and comparing blood work or function before and after, managed as a self-experiment 1.
Resveratrol: on the current human meta-analytic record, it is not worth the money under an anti-aging label 3.
How strong is the evidence
Randomized trials in people: the NR and NMN trials are double-blind 1112. Long-term NR in healthy middle-aged and older adults was well tolerated and effectively raised NAD metabolism 11. NMN improved muscle insulin sensitivity in women with prediabetes 12. A randomized, placebo-controlled trial of urolithin A reported about a 12 percent rise in strength, plus changes in endurance, the six-minute walk, and mitochondrial-related proteins. The primary endpoint, peak power, did not rise in a statistically clear way 13. What these show is an effect at the level of surrogate markers, not lifespan.
Meta-analysis: improvement from resveratrol across multiple metabolic-health markers was not supported 3.
Animal and mechanism studies: NAD precursors improve age-related phenotypes in several animal models 67. In an unpublished mouse pretest, the group given NR grew tumors faster. Each group had only 4 animals. It can only be read as a risk hint that has not been repeated 2.
Expert opinion and platform reviews help with sorting. They cannot replace any layer above.
What you can do
Build the free foundation first. Regular exercise, strength and aerobic, sleep, and a diet with less processing. That is the largest common factor across these views 45.
If NAD+ precursors or urolithin A are under consideration, a conversation with a clinician is the place, plus a trial spec you set for yourself: how long, which markers, and stop if nothing changes 1.
Food has natural sources. NR is naturally present in milk and beer. Broccoli, avocado, and beef contain NMN 68. Pomegranate, strawberry, and walnut supply ellagitannins, the raw material for urolithin A. Whether conversion happens, and how much, differs by person 10.
Readings already in hand that are worth discussing with a clinician: lipids such as apoB, HbA1c, and inflammation markers. These are both the domains supplements claim to affect and ready-made tracking tools.
For any supplement advertised as reversing aging, ask three things first. Is there a human randomized trial? Is the endpoint a surrogate marker or a health outcome? Who stands behind the claim 13?
Common misconceptions
Misconception one: if cellular NAD went up, the person got younger. What human trials have confirmed is a change in NAD level and in some surrogate markers. Lifespan and disease endpoints have never been shown 115.
Misconception two: resveratrol is the benefit of red wine, so a little alcohol tops it up. Red wine contains a trace of resveratrol. The compound's own evidence of metabolic benefit in people keeps failing, and the harm of alcohol is clear 35.
Misconception three: a supplement from a natural source has no risk. Human data at the level of tumor metabolism are still too thin. The direction hinted at by the mouse pretest needs to be taken seriously 2.
Misconception four: urolithin A works for everyone. It is produced by gut microbes, and people sort into different metabotypes. Taking the purified substance can bypass that difference. Individual response still depends on the starting state 10.
Misconception five: if an expert takes it, the expert is endorsing it as life-extending. Whether a prescription drug is thought to have a broad lifespan effect does not depend on whether someone takes it out of personal belief. Such judgments are not a general answer that applies to everyone 5.
How it connects to your blood work
No routine blood test directly measures NAD inside cells, or a "supplement effect." The markers below sit next to the domains these three supplements claim to affect, and they suit a before-and-after comparison in a self-experiment.
- Fasting glucose (fasting-glucose) and HbA1c (hba1c): what changed in the NMN trial was muscle insulin sensitivity. The glucose picture is a ready-made indirect window 12.
- Apolipoprotein B (apob) and LDL cholesterol (ldl-c): core readings for cardiovascular risk, and the domain supplement narratives most like to touch.
- High-sensitivity C-reactive protein (hs-crp): NR's anti-inflammatory narrative and the inflammation-marker changes in urolithin A trials both land here 105.
- Vitamin D (vitamin-d-25oh): on one supplement page it is written as a nutrient tied to bone, immunity, cardiovascular function, and hormone regulation. That is not the same narrative as NAD+ precursors. It is also one of the few markers where a shortfall and a refill can be checked objectively 4.
